Hypersensitivity to clindamycin or any component
Can cause pseudomembranous colitis and reversible liver enzyme elevation, do not give if previous pseudomembranous colitis or hepatic impairment

> 28 days old: 7.5 mg/kg IV q6h or 5-6 mg/kg PO q8h

IV: 300-900 mg q6-8h (max 2.7 g DIE)
