8-28 days old: 10 mg/kg IV/IM q12h
> 28 days old: 10 mg/kg IV/IM q 8h
Adults:
7.5 mg/kg IV q12h

Creatinine clearance (ml/min):
<10: 1.5-2.5 mg/kg IV q 24-48h
10-50: 2.5-5 mg/kg IV q12-18h
