AMIKACIN

Antibacterial Activity

Effective against gram-negative aerobes and mycobacteria

Contraindication/Precaution

Hypersensitivity to amikacin or any component
Use caution if impaired renal function

Mechanism of Action

Inhibits protein synthesis initiation by binding to the 30s ribosomal subunit

1.4-2.3 hours, change with renal function.

excreted  in urine

Dosage

Children: 

Body weight < 2000 g 0-7 days old: 7.5 mg/kg IV/IM q18-24h 8-28 days old: 7.5 mg/kg IV/IM q12h Body weight > 2000 g
0-7 days old: 10 mg/kg IV/IM q12h
8-28 days old: 10 mg/kg IV/IM q12h
> 28 days old: 10 mg/kg IV/IM q 8h

Adults:  

7.5 mg/kg IV q12h

Dosage In Renal Failure

Creatinine clearance (ml/min):
<10: 1.5-2.5 mg/kg IV q 24-48h 10-50: 2.5-5 mg/kg IV q12-18h 50-80: 4.5-7 mg/kg IV q12h >80: unchanged
Dialysis
Hemo: post dialysis dose: 5 mg/kg IV
CAPD: 2.5 mg/kg IV
