AZTREONAM

Antibacterial Activity

Effective against gram-negative only
Often useful in patients with allergies to penicillins or cephalosporins

Pregnancy Risk Factor

Category B

Contraindication/Precaution

Hypersensitivity to aztreonam or any component
Cross-allergenicity to penicillins and cephalosporins is possible

Mechanism of Action

Inhibits bacterial cell wall synthesis by binding to the penicillin binding proteins

Pharmacodynamics/Kinetics

Metabolism: 
liver
Half-life: 
1.7-2.9 hours, change with renal function.
Elimination: 
60% to 70% excreted unchanged in urine

Dosage

Children: IV

Body weight < 2000 g 0-7 days old:  30 mg/kg IV q 12h 8-28 days old:  30 mg/kg IV q 8h Body weight > 2000 g
0-7 days old:  30  mg/kg IV q 8h
8-28 days old:   30 mg/kg IV q 6h
> 28 days old: 30 mg/kg IV q 6h

Adults:  

1 g q6h or 2 g q8h

Dosage In Renal Failure

Creatinine clearance (ml/min):
<10: 1 g q24 or 500 mg q8h 10-50: 500 mg q12h or 1 g q8h 50-80: 1 g q8h or 2 g q12h >80: unchanged
Dialysis
Hemo: 500 mg
CAPD: 500 mg q8h
